A thin camera passed through the cervix to look inside the uterus, often removing polyps, fibroids or scar tissue in the same sitting, without any cuts.

Bleeding between periods, bleeding after menopause, or a scan report that says "thickened endometrium" without saying why. For other women it's a string of failed pregnancy tests or miscarriages. In each case the answer often lies inside the uterine cavity, which an ultrasound can only partly show.
A hysteroscopy uses a thin telescope with a light and camera (a hysteroscope), passed through the vagina and cervix into the uterus. Sterile fluid gently opens up the cavity so the doctor can see the lining clearly on a screen. In a diagnostic hysteroscopy the doctor looks and may take a small sample. In an operative hysteroscopy, fine instruments passed through the scope remove polyps, fibroids, a septum or scar tissue.
There are no cuts on your abdomen, and most women go home within a few hours. The doctor can usually tell you what was seen the same day, and any biopsy report follows in about a week. Many problems, a polyp for example, are found and fixed in one go.
Hysteroscopy has its own dedicated suite in our gynaecology department. We use it to investigate problems inside the uterus and to treat fibroids and other growths in the cavity, often in the same visit.
Hysteroscopy is the most direct way to see the inside of the uterus. It's usually suggested after an ultrasound raises a question or doesn't explain your symptoms.
A very thin scope is used, often with no anaesthetic or just a numbing injection into the cervix, as an outpatient procedure. It takes about 10 to 15 minutes, and most women feel period-like cramps.
Polyps, fibroids, a septum or scar tissue are cut or shaved away with fine instruments or an electrical loop (resectoscope). It's usually done under general anaesthesia or sedation as a day-care procedure, taking 20 to 45 minutes.
When infertility is being investigated, hysteroscopy is sometimes combined with a laparoscopy in the same sitting, to check the outside of the uterus, the tubes and the ovaries as well.
You lie on your back with your legs supported, as for a pelvic examination. A female nurse or attendant is with you throughout.
The scope is slid gently through the cervix, sometimes after the cervix is numbed or slightly widened. No cut is made.
Sterile fluid flows through the scope to open up the cavity. The doctor checks the lining, the shape of the cavity and the openings of both fallopian tubes.
If needed, polyps, small fibroids, a septum or scar tissue are removed, and a sample of the lining is taken. The fluid going in and coming out is measured throughout.
The scope is withdrawn and you rest for an hour or two. A diagnostic hysteroscopy takes about 10 to 15 minutes, and an operative one 20 to 45.
You go home within a few hours. Period-like cramps and light bleeding or watery discharge are normal. A hot water bottle and regular painkillers help.
Most women go back to work the next day after a diagnostic hysteroscopy, or after 2 to 3 days following an operative one.
Avoid sex, tampons and swimming until the bleeding stops, usually about a week, to lower the risk of infection. Your biopsy report and next steps are discussed at follow-up.
If a septum or scar tissue was removed, your doctor may suggest a short course of hormones to help the lining regrow, and sometimes a check hysteroscopy, before you start trying.

Fluid is put into the uterus during a vaginal ultrasound. It shows polyps and fibroids well, but it can't treat them or take a targeted sample.
A thin tube takes a sample of the lining in a few minutes. It's quick but done without seeing inside, so it can miss a polyp or a small area of change.
A D&C scrapes the lining without a camera. It has largely been replaced by hysteroscopy, which lets the doctor see what is being removed.
