A small operation that joins an artery to a vein in your arm, so the vein grows strong enough for dialysis needles and serves you for years.

If your kidney function is sliding towards dialysis, one of the most useful things you can do now has little to do with your kidneys. It is protecting the veins in one arm and getting a fistula made in good time, months before you need it.
An arteriovenous (AV) fistula is a direct connection, made by a surgeon, between an artery and a vein, usually at the wrist or elbow of the arm you don't write with. Blood from the artery rushes into the vein at high pressure. Over the following weeks the vein's wall thickens and the vein widens, until it can take two dialysis needles three times a week and deliver the fast flow a dialysis machine needs. You can feel it under the skin as a buzz, called the thrill.
A fistula uses only your own blood vessels, so it clots and gets infected less often than a plastic graft or a neck catheter, and a good one can last many years. The catch is time. Most fistulas need 6 to 8 weeks, often longer, before they can be used. Some never mature enough, particularly in older people, people with diabetes and those with small veins, and need a second procedure or a new fistula higher up the arm.
Our nephrologists talk through dialysis options early with people approaching end-stage kidney disease, which leaves time to plan access before dialysis becomes urgent. Once your fistula is ready, it's used for haemodialysis in our dialysis unit, where trained nephrology nurses check it at every session and a nephrologist reviews you each time.
A fistula is the first-choice access for almost everyone starting long-term haemodialysis.
The radial artery is joined to the cephalic vein at the wrist. It's usually tried first, because it keeps the veins higher up the arm free for later. It is more likely to fail to mature, especially with small vessels.
The brachial artery at the elbow is joined to the cephalic vein. It carries more blood and matures more reliably, so it's the usual choice when wrist vessels are too small, in older people and in many people with diabetes. The risk of the hand being starved of blood (steal) is higher.
The basilic vein lies deep in the upper arm, so it has to be moved closer to the surface where needles can reach it, often in a second operation a few weeks later. It's kept for when simpler options have been used up.
Most fistulas are made under local anaesthetic or a nerve block that numbs the whole arm (brachial plexus block), with light sedation if you need it.
The surgeon makes a cut of about 3 to 5 cm at the wrist or elbow, over the chosen artery and vein.
The vein is freed, divided and swung across to lie beside the artery. A small opening is made in the artery, and the vein is stitched to it with very fine stitches, often under magnification.
When the clamps come off, the surgeon feels for the thrill and listens for the whooshing sound (bruit) that shows blood is flowing through the new connection.
The skin is closed with stitches and a light dressing applied. Most people go home the same day.
The wound heals in about 2 weeks. The fistula takes longer to become usable, and a few daily habits help it along.
Keep the arm raised on a pillow to limit swelling, and check for the buzz several times a day. Numbness from a nerve block wears off over some hours.
Keep the dressing dry. Stitches come out at 10 to 14 days. Some swelling and bruising is normal. Don't lift anything heavier than about 2 kg with that arm.
Start hand exercises when your doctor says, such as squeezing a soft ball or rolled towel for a few minutes several times a day, which may help the vein grow. Your nephrologist examines the fistula and may repeat an ultrasound to see whether it's maturing.
When it's ready, often after 6 to 8 weeks, the first needling is done gently, sometimes with smaller needles and lower flows at first. After that, a daily check of the thrill and protecting that arm keep the fistula working for years.

A synthetic tube joins an artery and a vein when your veins are too small for a fistula. It can be used within 2 to 3 weeks, but it clots and gets infected more often.
A tube placed in a large neck vein and tunnelled under the skin can be used straight away. Infection and blockage are more common, so it's a bridge until a fistula or graft is ready.
Needs no blood vessel access at all. Fluid is exchanged at home through a soft tube in the abdomen. Read about peritoneal dialysis.
