Overview

People often put it the same way: "I look angry, or tired, and I'm not." Usually it's the vertical lines between the eyebrows, or cheeks that have lost some fullness so the folds beside the mouth look deeper. Both can be softened with injections, without surgery.

The two treatments work differently. Botulinum toxin (widely known by the brand name Botox) is a purified protein injected in tiny doses into chosen muscles. It blocks the nerve signal to those muscles so they relax, and the lines they crease into the skin smooth out. Dermal fillers are gels, most often hyaluronic acid (a sugar found naturally in skin), placed under the skin to replace lost volume, lift a fold or reshape an area such as the lips or chin.

Neither is permanent. Botulinum toxin usually starts working in 3 to 5 days, peaks at about 2 weeks and wears off over 3 to 4 months. Hyaluronic acid fillers last roughly 6 to 18 months and can be dissolved if needed. A good result makes you look rested, not "done". This page covers cosmetic use. For medical uses of botulinum toxin, such as chronic migraine and muscle stiffness after a stroke, see Botox for neurology.

Anti-ageing treatments are part of the cosmetic work of our skin and hair department, carried out by trained dermatologists. Before any injection we look at your face at rest and in movement, explain what the treatment can realistically do and how long it will last, and say so if we think you don't need it.

Who it helps

Botulinum toxin treats lines caused by movement. Fillers treat lines and hollows that are there even when your face is still.

  • Frown lines between the eyebrows, the '11' lines
  • Horizontal forehead lines
  • Crow's feet at the outer corners of the eyes
  • Deep folds from the nose to the corners of the mouth (nasolabial folds), and flattened cheeks
  • Thin lips, or lips that have lost their shape with age
  • A small or receding chin, or a softening jawline

Is it right for you?

Usually a good fit

  • Specific lines or hollows bother you and you are in good general health
  • You want a subtle change and accept that repeat treatments keep it going
  • The decision is your own, not made under pressure from others

May not be suitable

  • Pregnancy or breastfeeding
  • Myasthenia gravis or another nerve-muscle disease, for botulinum toxin
  • Infection, cold sores or pimples at the injection site, until they clear
  • A past severe allergic reaction to a filler or botulinum toxin
  • Heavy, drooping eyelids or brows, which botulinum toxin can make worse and surgery may treat better

Which injection does what

Botulinum toxin for expression lines

A few tiny injections go into the muscles that frown, lift the brows or squint. The dose decides how much movement is left. Most people prefer a light touch, so they can still show expression.

Hyaluronic acid fillers

Fillers come in different thicknesses. Firmer gels go deep, near the bone, to lift the cheeks and chin, and softer gels suit the lips and fine lines. Because an enzyme called hyaluronidase can dissolve hyaluronic acid, it is the safest filler type for most people. Many faces need both treatments: botulinum toxin for frown lines and a filler for flattened cheeks.

Before you start

  • Have a consultation first. If lost volume is the concern, bring photos of yourself from a few years ago
  • Tell us about any nerve or muscle condition, bleeding problem or allergy, and all your medicines. Some antibiotics, such as gentamicin, can strengthen the effect of botulinum toxin
  • If you take aspirin, clopidogrel or another blood thinner, don't stop it yourself. Ask the doctor who prescribed it
  • Skipping alcohol for 24 hours, and painkillers such as ibuprofen or fish oil supplements for about a week, cuts down bruising, if your doctor agrees
  • Keep 2 weeks clear before a wedding, festival or photo shoot. Bruises need time to fade, and botulinum toxin takes up to 2 weeks for its full effect
  • Avoid dental treatment for about 2 weeks before and after fillers, to lower the risk of infection

What happens at the appointment

  1. 01

    Planning

The doctor studies your face at rest and while you frown, smile and raise your brows, marks the injection points and takes photographs to compare later.

  2. 02

    Numbing and cleaning

The skin is cleaned with antiseptic. For fillers, a numbing cream goes on 20 to 30 minutes beforehand, and many filler gels also contain a local anaesthetic.

  3. 03

    Injecting

Botulinum toxin is given through a very fine needle in a few small pricks, which takes minutes. Filler is placed with a fine needle or a blunt-tipped tube (cannula) a little at a time, while the doctor shapes it and checks symmetry.

  4. 04

    Checking and aftercare

Ice reduces swelling. You sit upright for a few minutes, receive written aftercare advice and can go home or back to work.

Benefits and risks

Benefits

  • Softens frown lines, forehead lines and crow's feet without surgery
  • Restores lost volume in the cheeks, lips, temples or chin
  • Botulinum toxin works within days, and fillers show immediately
  • No stitches or hospital stay, and most people return to work straight away

Possible risks

  • Small bruises and swelling at the injection points, which settle over a few days to a week
  • A drooping eyelid or eyebrow if the toxin spreads to a nearby muscle. It is uncommon and wears off over a few weeks
  • Lumps, unevenness or a bluish tinge under thin skin after filler, which can usually be massaged or dissolved
  • Rarely, filler blocks a blood vessel, which can damage the skin and, very rarely, affect sight. It needs emergency treatment with the dissolving enzyme, one reason injections belong in trained medical hands
  • Infection or delayed swelling around filler weeks or months later, which is uncommon

After your injections

Your main job afterwards is to leave the treated area alone.

  1. 01

    The first 4 hours

After botulinum toxin, stay upright and don't press, rub or massage the area, so the toxin stays where it was placed. Avoid lying flat or bending down for long.

  2. 02

The first 24 to 48 hours

Skip workouts, saunas and steam. After fillers, avoid alcohol for a day, sleep with your head slightly raised if there is swelling and don't press on the area. Make-up can go on the next day.

  3. 03

    Days 3 to 14

By 2 weeks botulinum toxin has reached its full effect and filler swelling has settled, so this is when you see the real result. A review then allows small top-ups.

  4. 04

    The months after

Movement returns gradually after 3 to 4 months, and fillers soften over 6 to 18 months. Repeat treatment happens when you want it, not on a fixed timetable.

When to call your doctor

  • Skin that turns white, grey, blue or blotchy, or severe pain, during or soon after a filler injection
  • Any change in vision, eye pain or a severe headache after filler near the eyes or nose
  • Difficulty swallowing, speaking or breathing, or general muscle weakness after botulinum toxin, which is very rare with cosmetic doses
  • Increasing redness, heat, pain or a hard lump days to weeks after filler

Other options

Skin treatments for fine lines

Fine lines, rough texture and pigmentation respond better to a chemical peel, microneedling or laser than to injections. Daily sunscreen does more to slow skin ageing than any single procedure.

Surgery

For heavy upper eyelids, loose neck skin or deep jowls, a surgical lift achieves more than injections can. A plastic surgeon can tell you whether it makes sense for you.

Leaving it alone

Lines and volume loss are part of normal ageing, not a health problem. Not treating them is always a reasonable choice.

Frequently Asked Questions

Answers to common questions about Botox and Dermal Fillers at Prakash Hospital, Noida.

Will I look frozen or fake?

Not if the dose and placement are planned with care. The aim is to soften lines and keep expression. We usually start conservatively and add a little at the 2-week review if needed, which is easier than undoing too much.

Is there a right age to start?

There's no set age. Some people start small doses of botulinum toxin in their late 20s or 30s for early frown lines, while fillers are more often used from the 40s, as volume is lost. Cosmetic injections are for adults only.

Isn't botulinum toxin a poison?

It comes from the same bacterium that causes botulism, but it is purified and used in doses far smaller than those that cause harm, and it has been used in medicine for decades. In trained hands, side effects are usually mild and temporary.

Can I have Botox or fillers while pregnant or breastfeeding?

No. There isn't enough safety information, so we advise waiting until you've finished breastfeeding.

What happens if I stop?

Your face gradually goes back to how it would have looked anyway. Lines don't get worse because you had treatment. Some people find the lines have deepened more slowly, because the muscle was resting.

Does insurance cover cosmetic Botox or fillers?

Cosmetic treatments are generally not covered. Botulinum toxin for a medical condition such as chronic migraine may be, depending on your policy. Our empanelled insurers and TPAs are listed online.
