Men's Mental Health in India: The Signs We Keep Missing

by By Prakash Hospital

A man looking stressed at his workplace

In men, depression often shows up as irritability, overwork or heavier drinking rather than sadness.

Ask an Indian man how he is, and the answer is almost always "fine." Ask again a week later, after he has stopped sleeping, snapped at his family, started drinking more, or gone silent at work, and the answer is still "fine."

Men in India are raised to carry responsibility without complaint. Be the earner. Be the strong one. Don't cry. The result is a population of men who experience depression, anxiety, and burnout at real rates but rarely name them, rarely seek help, and too often reach a crisis before anyone notices.

India reports a far higher number of male suicides than female suicides each year, and men account for roughly two-thirds of all suicide deaths in the country. That figure alone says that something is going unspoken.

Why Men Under-Report Mental Health Problems

Stigma around weakness. Mental health problems are still seen by many as a personal flaw rather than a medical condition. Men fear being judged by family, colleagues, or in-laws.

The provider role. Many men feel they cannot afford to stop, slow down, or admit struggle because others depend on them financially.

Different symptoms. Depression in men often does not look like sadness. It looks like anger, irritability, risk-taking, withdrawal, or physical complaints. Family members and even doctors may miss it.

Alcohol as self-medication. Drinking, smoking, or tobacco chewing is a common way of numbing distress, and it makes the underlying problem worse.

Limited vocabulary. Many men have never been taught to describe feelings, so they describe the body instead: "my head feels heavy," "I'm tired all the time."

Common Conditions in Indian Men

Depression

Persistent low mood, loss of interest in things once enjoyed, fatigue, poor sleep or sleeping too much, appetite change, difficulty concentrating, feelings of worthlessness, and in severe cases thoughts of death. In men it frequently shows up as:

  • Irritability and short temper
  • Working constantly to avoid thinking
  • Increased alcohol use
  • Headaches, body aches, stomach problems with no clear cause
  • Loss of sexual interest

Anxiety

Constant worry, restlessness, a racing heart, tight chest, sweating, trouble sleeping, and a sense that something bad is about to happen. Many men with anxiety attacks end up in the emergency department convinced they are having a heart attack.

Burnout and work stress

Long hours, job insecurity, commute, and constant connectivity wear down even resilient people. Burnout shows up as exhaustion, cynicism about work, and reduced performance. It overlaps with depression and often leads to it.

Substance use

Alcohol, tobacco, gutka, and other substances are used to cope. Dependence develops gradually. A man who needs a drink to sleep or relax each evening has crossed a line worth discussing with a doctor.

Suicidal thoughts

These can occur in depression, after financial loss, relationship breakdown, or job loss, and sometimes without an obvious trigger. They are a medical emergency, not a sign of weakness, and help is available.

Warning Signs to Watch For

In yourself or a man you love, watch for changes lasting more than two weeks:

  • Withdrawing from family and friends
  • Losing interest in hobbies or work
  • Sleeping much more or much less
  • Drinking more than before
  • Unusual anger or aggression
  • Neglecting responsibilities or appearance
  • Talking about being a burden or about not wanting to be here
  • Giving away possessions or saying goodbye in unusual ways

If someone talks about ending their life, take it seriously. Stay with them, remove access to means of harm, and get professional help immediately. In India, the national Tele-MANAS helpline (14416) offers round-the-clock mental health support. If there is immediate danger, go to the nearest emergency department.

Mental health and physical health are bound together. Chronic stress raises blood pressure, blood sugar, and cardiovascular risk. Depression is linked to poorer outcomes in heart disease and diabetes. Low testosterone, thyroid disease, vitamin D and B12 deficiency, and sleep apnoea can all mimic or worsen depression. A proper medical evaluation checks for these, which is one reason to start with a doctor rather than guessing.

What Helps

Talk to someone

This is the hardest step and often the most effective. Start with whoever feels safest: a friend, sibling, spouse, or doctor. You do not need the right words. "I haven't been myself lately" is enough.

Professional treatment

  • Psychotherapy: cognitive behavioural therapy (CBT) is well proven for depression and anxiety. It teaches practical skills for managing thoughts and behaviour.
  • Medication: antidepressants and anti-anxiety medicines work well for moderate to severe conditions. They are not addictive in the way many fear, and they are prescribed and monitored by a doctor.
  • Combined care: for many men, therapy plus medication gives the best result.

Daily habits that support recovery

  • Move daily. Regular exercise has an antidepressant effect comparable to medication in mild to moderate depression.
  • Protect sleep. Fixed sleep and wake times matter more than people expect.
  • Cut back on alcohol. It worsens mood and anxiety, even when it feels like relief.
  • Stay connected. Isolation deepens depression. Even a short daily conversation helps.
  • Get sunlight and fresh air. Morning light supports mood and sleep rhythm.
  • Practise breathing and mindfulness. Ten minutes of slow breathing lowers physical tension.
  • Limit news and social media when they feed worry.

Workplace and financial stress

If work is the main source, talk to a manager or HR where possible, set boundaries on after-hours messages, and take your leave. For financial stress, speaking to a trusted adviser reduces the sense of being trapped.

How to Support a Man Who Is Struggling

  • Choose a private, calm moment and ask directly: "I've noticed you seem low. How are you really doing?"
  • Listen without fixing, lecturing, or comparing.
  • Avoid phrases like "be a man" or "others have it worse."
  • Offer to go with him to see a doctor.
  • Check in regularly, not just once.
  • If he mentions suicide, ask plainly and stay with him; asking does not plant the idea.

Myths That Keep Men Away From Help

"Therapy is for weak people." It is a skill-building process, used by people in demanding roles everywhere.

"Medication will change who I am." Properly prescribed medication helps you function as yourself again.

"People will find out." Consultations are confidential.

"It will pass by itself." Sometimes mild low mood does. Depression and anxiety that last weeks usually do not, and early treatment shortens recovery.
